How to apply for a certification:
L1
- Attend and pass# the exam (every 2 years). The minimum passing grade is 90% (registration here)
- Please write me an email with the subject: L1 Certification: YOUR_LASTNAME
- Please provide the following information:
- Picture of your First Aid Certification or FAC (not older than 3 years)
- Picture of your contract with UR, UKR or Medbo: at least SHK or WHK contract with the UKR, UR, Medbo (min. 12 months).
- Continuance of Employment is not mandatory!
- Please collect your (10 hrs) training sessions al L1 using the provided form
- include your sponsor (Principal Investigator) in the e-mail (CC)
- watch the Siemens Safety video
- Make an appointment with the SM for the final examination at the Prisma
- Please review all safety slides, as I will ask specific questions related to the course on that day
L2
- Attend and pass# the exam/refresher (every 2 years). The minimum passing grade is 90% (registration here)
- Please write me an email with the subject: L2 Certification: YOUR_LASTNAME
- Please provide the following information:
- Picture of your First Aid Certification or FAC (not older than 3 years)
- Picture of your contract with UR, UKR or Medbo: at least SHK or WHK contract with the UKR, UR, Medbo (min. 12 months)

L2-Direct
- Attend and pass# the L1 & L2 Tests. Refresher (every 2 years). The minimum passing grade is 90% (registration here)
- Please write me an email with the subject: L2-Direct Certification: YOUR_LASTNAME
- Please provide the following information:
- Picture of your First Aid Certification or FAC (not older than 3 years)
- Picture of your contract with UR, UKR or Medbo: at least SHK or WHK contract with the UKR, UR, Medbo (min. 12 months)
- Continuance of Employment is not mandatory!

#If you do not pass the exam or refresher, you will be temporarily unable to work at Prisma as L1/L2. You must wait until you pass the exam in the next Safety Training
