- Attend and pass# the exam (every 2 years)
- Write me an e-mail with the following information:
- Picture of your FAC (not older than 3 years)
- Picture of your contract with UR, UKR or Medbo: at least SHK or WHK contract with the UKR, UR, Medbo (min. 12 months)
- .xlsx file demonstrating the hrs (at least ten) you worked at the Prisma while helping with the experiments (DATE, HRS)
- include your sponsor (Principal Investigator) in the e-mail (CC)
- watch the Siemens Safety video
- Make an appointment with the SM for the final examination at the Prisma
L2
- Attend and pass# the refresher (every 2 years)
- Write me an e-mail with the following information:
- Picture of your FAC (not older than 3 years)
- Picture of your contract with UR, UKR or Medbo: at least SHK or WHK contract with the UKR, UR, Medbo (min. 12 months)
- .xlsx file demonstrating the hrs (at least ten) you worked at the Prisma while helping with the experiments (DATE, HRS) as L2
- include your sponsor (Principal Investigator) in the e-mail (CC)
- prove your 5-day training with the MTAs
- watch the Siemens Safety video
- Make an appointment with the SM for the final examination at the Prisma
#If you do not pass the exam/refresher, you are temporarily not allowed to work at Prisma as L1/L2. You have to wait until you pass the exam in the following Safety Training
